This is how the hormonal acne story unfolds: Just when we think puberty’s passing took unsightly teenage acne with it, we’re introduced to a new wave of pimples in our 20s. Right around the time of our periods, acne flares up along our jawline, cheeks, forehead, and between the eyebrows , adding insult to injury. Looking on the bright side, though, you have options when it comes to helping quell hormonal acne. But let’s first dive into how to identify them.

The Best Skin Care Routine For Acne

Whether youve been dealing with acne for years or have unexpectedly found yourself struggling with new breakouts, I feel your pain. Starting in my mid-30s, hormonal shifts caused my previously unblemished skin to suddenly freak out. After a lifetime of clear skin , it felt like a slap in the face. After all, battling both wrinkles and acne at the same time feels utterly unfair.

Few things are more demoralizing than adult acne. It feels like a betrayal when your perfectly honed routine all of a sudden starts to fail you, and figuring out a new balance of products and ingredients can be confounding. Thats why were talking to experts about adult acne and the best skin care routine for those of us with acne-prone skin.

Dr. Y. Claire Chang, a board-certified cosmetic dermatologist at Union Square Laser Dermatology in New York City, gave some insight into the very frustrating causes of adult acne. She explained that, much like my own experience, adults can get acne from their 30s to 50s even if they didnt have teenage acne. However, the underlying causes of adult acne are actually similar to teenage acne, and include excess oil production, clogged pores, bacteria and inflammation.

Other causes include hormonal fluctuations, stress, diets that are high in sugar, carbs and dairy, heavy alcohol consumption and mask use.

What Are The Best Oral Contraceptives For Women With Hormonal Acne

We know that dealing with hormonal acne can be super frustrating, so the idea of taking a birth control pill sounds like an appealing way to help to regulate hormones and get breakouts under control. However, before you start taking birth control pills, you should know that not all contraceptives work the same way and that some birth control pills can actually make acne worse. Seriously. Talk about a bummer. But knowledge is power, and here’s what you need to know.

The best oral contraceptives to control hormonal or adult acne are the ones that contain Ethinyl estradiol and one of the following: drospirenone, norethindrone, or norgestimate. Beware that oral contraceptives are contraindicated in women with a history of blood clots, high blood pressure, breast cancer, and women that smoke.

A study recently published in the Journal of Drugs in Dermatology categorized the different types of contraceptives by their efficiency in clearing acne. These are the birth control pills the study said significantly improve hormonal acne: Yaz, Ocella, Yasmin, Trinessat, MonoNessa, Apri, and Reclipse. Pretty long list, right?

There are also some contraceptives that the study found to have little or no effect on acne. Those are Nuvaring, Microgestin pills, and Orthoevra . The study also found that some contraceptives can actually make acne WORSE. That list includes Depo-Provera , Skyla, Lylema, Implanon, and Nexplanon .

Our beauty editor shares her personal story.

By Hannah Zahner

My skin loved being pregnant, but around sixth months postpartum, it took a turn for the worse. While Ive dealt with mild forehead breakouts off and on since college, I never had acne or breakouts on my jaw and cheekuntil this past year. On top of that, I am generally on top of my skincare game. I never ever go to bed without washing my faceI can count on one hand the times I havent washed my face before bed in my entire life. I eat a clean and nourishing diet, use organic high-powered skincare, and put a lot of effort into supporting my health holistically.

Although it seemed like my skin should be okay, I was still dealing with hormonal acne, and my current routine wasnt working.

While Im obsessed with skincare, I dont love the idea that you have a lineup of 20 serums to get good results. In my quest to calm my skin, I had carts filled with hundreds of dollars worth of skincare, ready to pull the trigger to do whatever I could to clear my skin up. Thankfully, I took a breather and remembered that theres more to skin health than just throwing products at it. Skin is a reflection of your internal health and while yes, Ive found new products that have supported my skins health along the way, there were some major non-product-related steps that were crucial when figuring out how to clear up hormonal acne.

Scroll on for what Ive learned, both internally and topically with products.

The severity of the acne may influence the type of treatment a person chooses, and people with more severe breakouts may prefer prescription oral medications or topical treatments, such as creams or gels.

A person may use acne treatment for at least 46 weeks before they see results. It is best to avoid using multiple acne treatments, to prevent irritation and new breakouts.

Hormonal acne treatments fall into the four categories below.

Self-care techniques that can help clear mild and moderate hormonal acne breakouts include:

  • avoiding exfoliating face washes, astringents, and masks
  • using noncomedogenic, oil-free skin care products
  • washing the face twice a day, once after waking up and again before going to bed
  • washing the face and showering after workouts
  • avoiding picking, touching, or popping acne
  • shaving gently and only when necessary
  • using sunscreen

There is debate about a possible link between the diet and acne. Some studies suggest that certain foods can worsen acne, while others can be beneficial. However, researchers have yet to make definitive conclusions.

What Is Fungal Acne Vs Hormonal Acne

Fungal acne and hormonal acne are often confused with each other because both types of acne start in the hair follicles. Fungal acne occurs when excess yeast develops, whereas hormonal acne is caused by excess sebum. Fungal acne can result in whiteheads, itchiness and often becomes red, irritated and inflamed.

Take An Oral Antibiotic Or Medication

While topicals can be great, they are limited in how much work they can do when it comes to breakouts. Antibiotics are ingested, circulating through the body and targeting the root cause of breakouts: the sebaceous glands. “Trying a different combined oral contraceptive pill may also help, as can oral antibiotics or a medication called spironolactone, which has been used off-label for years to help treat hormonal acne in adult women,” says Granite.

Other commonly prescribed antibiotics for acne include doxycycline and minocycline, which are meant to help lessen inflammation and therefore breakouts. It’s worth noting that antibiotics can lead to resistance, so they shouldn’t be used forever, and some people might develop side effects as a result of adding them to their routine. They can also be used in conjunction with topical products, such as retinol or benzoyl peroxide, or alongside procedures like light and laser therapy as well as chemical peels.
