In two separate large population studies, both benzodiazepines and anticholinergics were associated with an increased risk of dementia in people who used them for longer than a few months. In both cases, the effect increased with the dose of the drug and the duration of use.

These findings didn’t come entirely as a surprise to doctors who treat older people. The Beer’s List published by the American Geriatrics Society has long recognized benzodiazepines, antihistamines, and tricyclic antidepressants as potentially inappropriate for older adults, given their side effects. Such drugs are on the list because they share troubling side effectsconfusion, clouded thinking, and memory lapsesthat can lead to falls, fractures, and auto accidents.

Where To Learn About Other Drugs That Affect Brain Function

Many other drugs that affect brain function, but they are either not used as often as the ones above, or seem to affect a minority of older adults.

Notably, there has been a lot of concern in the media about statins, but a meta-analysis published in 2015 could not confirm an association between statin use and increased cognitive impairment. In fact, a 2016 study found that statin use was associated with a lower risk of developing Alzheimers disease.

This is not to say that statins arent overprescribed or riskier than we used to think. And its also quite possible that some people do have their thinking affected by statins. But if you are trying to eliminate medications that dampen brain function, I would recommend you focus on the ones I listed above first.

  • Commonly prescribed opiates include hydrocodone, oxycodone, morphine, codeine, methadone, hydromorphone, and fentanyl.
  • Tramadol is a weaker opiate with weaker prescribing controls.
  • Many geriatricians consider it more problematic than the classic Schedule II opiates listed above, as it interacts with a lot of medications and still affects brain function. Its a dirty drug, as one of my friends likes to say.
  • Alternatives depend on what type of pain is present. Generally, if people are taking opiates then they have pain that needs to be treated. However, a thoughtful holistic approach to pain often enables a person to get by with less medication, which can improve thinking abilities.
  • For people who have moderate or severe dementia, its important to know that untreated pain can worsen their thinking. So sometimes a low dose of opiate medication does end up improving their thinking.
  • Other risks in older adults:
  • There is some risk of developing a problematic addiction, especially if theres a prior history of substance abuse. But in my experience, having someone else usually younger steal or use the drugs is a more likely problem.

    If you are taking an over-the-counter sleep aid, it contains a sedating antihistamine and those are strongly anticholinergic. You can just stop an OTC sleep aid, but in the short term, insomnia often gets worse. So youll need to address the insomnia with non-drug techniques.

    You should also discuss any insomnia or sleep problems with your doctors its important to rule out pain and serious medical problems as a cause of insomnia but be careful: many of them will prescribe a sleeping pill, because they havent trained in geriatrics and they under-estimate the risks of these drugs.

    What The Studies Found

    It’s important to note that neither of these studies was a randomized controlled clinical trial, so neither proved that either type of drug causes dementia.

    The anticholinergic study. Researchers tracked nearly 3,500 men and women ages 65 or older who took part in Adult Changes in Thought , a long-term study conducted by the University of Washington and Group Health, a Seattle health care system. They used Group Health’s pharmacy records to determine all the drugs, both prescription and over-the-counter, that each participant took in the 10 years before starting the study. Participants’ health was tracked for an average of seven years. During that time, 800 of them developed dementia. When the researchers examined medication use, they found that people who used anticholinergic drugs were more likely to have developed dementia than those who didn’t use them. Moreover, dementia risk increased along with the cumulative dose. Taking an anticholinergic for the equivalent of three years or more was associated with a 54% higher dementia risk than taking the same dose for three months or less.

    The University of Washington study is the first to include nonprescription drugs. It is also the first to eliminate the possibility that people were taking the drugs to alleviate early symptoms of undiagnosed dementia. For people who took anticholinergic bladder medications, the increased risk was just as high as for those taking tricyclic antidepressants, which are also anticholinergics.

    Who Should Take Sleeping Pills

    When a person has insomnia, they may have trouble falling or staying asleep, wake up too early, or not feel refreshed upon waking.

    However, many people may not need sleeping pills to treat insomnia. Lifestyle adjustments, such as exercising and avoiding alcohol and nicotine, can often improve a persons sleep cycle and prevent insomnia. Changing the sleep environment, such as replacing an old mattress, might also help.

    If making these adjustments does not help treat severe insomnia, a sleeping pill may be a good short-term solution.
