As a reminder, were talking about how much melatonin is safe for kids.

Most sleep supplements dont have less than 1mg of melatonin, which Ive already acknowledged is significantly more than even adults need over the course of a full night.;

Since adults naturally create about 6µg per night, kids likely make around 3µg per night.

I cant give specific details on the Sleep Remedy formula because its proprietary, but I can say that the amount of melatonin in this product is around 250µg.

That means this is the only sleep supplement for kids that I know of which likely results in anything close to the amount of melatonin a kid needs as hes initiating sleep.

Delayed sleep phase syndrome is a common disorder in teens, where their natural sleep period is shifted significantly later than the schedule which their commitments mandates. Thus, teens with this disorder an unable to fall asleep by 12 AM in the morning or even later. I have seen kids who are routinely falling asleep between 45 AM. Melatonin has a clear role in this disorder, as small doses 34 hours earlier than sleep onset can be effective in managing this disorder. Pediatricians ;frequently recommend melatonin for children with sleep problems, or parents might try it themselves. However, the proper use of melatonin is frequently misunderstood. Here is a guide for parents and pediatricians to decide if a child should try it, and to understand how it should be used.

A common thread I find in children coming to Sleep Clinic is that many or all of them have been on melatonin at some point, or are taking it currently. Melatonin is an important tool in the treatment of sleep disorders in children, and because it is naturally derived, there is a widespread perception that it is safe. However, I have become concerned by the frequency of its use, especially in an unsupervised way.

Melatonin sales have doubled in the past ten years,;increasing;from $90 million in 2007 to $260 million in 2012. Melatonin for children products are being marketed aggressively.;;I worry that the widespread availability of melatonin has led to some parents using it as a shortcut to good sleep practices. An article in the Wall Street Journal , quoted a fathers review on Amazon:

OK, yes, as parents my wife and I should do a better job starting the bedtime routine earlier, turning off the TV earlier, limiting sweets, etc., etc. Well, for whatever reason, this is not our strong suit. Why Does My Child Have A Sleep Problem

Sleep problems are common in children, especially those children with neurodevelopmental disorders . A range of things can cause difficulty falling asleep or staying asleep. It is important to find out the reasons for your child’s sleep problems. Knowing the reason means you can use the right tools to manage the sleep problem.

Pain, reflux, sleep-disordered breathing and worrying can all have an effect on your child’s sleep. Make sure your child receives a diagnosis and treatment for these before thinking about using other strategies such as a trial of melatonin.

The plasma half-life is quite short and ranges between 20 and 50 minutes. This means that approximately only half of the dose remains in your system after this amount of time. This means that while it may help you fall asleep faster, it likely wont help you to stay asleep or increase the quality of your sleep.

How Do You Know If Melatonin Might Be Helpful

Children on melatonin should meet some criteria for insomnia, and in particular for difficulty falling asleep. But they also should have other things ruled out, experts said.

If you are worried about your kids sleep, talk to your pediatrician, said Craig Canapari, M.D., an associate professor of pediatrics at Yale University, director of the Pediatric Sleep Center at Yale-New Haven Hospital. Melatonin certainly may be on the table for a finite period of time, but you have to look and see is there an underlying problem that is causing your childs insomnia.

For example, melatonin is not appropriate for restless leg syndrome. Both the American Academy of Pediatrics and the National Institutes of Health say that children should not take melatonin long-term, but neither organization defines what long-term means. Some children naturally fall asleep later than parents might like, but that doesnt mean they need melatonin.

Teach Your Child Healthy Bedtime Habits

Whether or not you choose to give your child melatonin as a sleep aid on a limited basis, having a regular bedtime routine should make it easier for your child to fall asleep.

Try these ideas:

  • establish a set bedtime, so your child expects to go to sleep at the same time every night
  • have a set wakeup time each morning, so your child is tired at bedtime
  • put away smartphones and tablets an hour before bed, and keep devices out of the bedroom
  • create a relaxing bedtime routine bathing, reading and listening to music are good options
  • keep your childs bedroom cool and dark
  • expose your child to bright light in the morning

If your child often has trouble falling asleep, talk to the pediatrician about possible causes of the problem, rather than giving your child melatonin gummies regularly.

The supplements we see on the market today are far more than what we need even as adults.

Melatonin isnt just produced in the brain. Its produced all over the body.;

Its also not just a hormone its also an antioxidant that has all sorts of useful functions throughout the body besides just sleep.

But, were talking about sleep here. So, lets get into it.;

Melatonin Dosage For Older Adults

Our melatonin levels naturally , disrupting the sleep-wake cycles for many older adults. As a result, older adults may have an increased sensitivity to melatonin. In a meta-analysis of 16 studies, melatonin dosages between 0.1 milligram and 50 milligrams per kilogram were administered to older adults aged 55 to 77 years old. In all of the studies, the melatonin levels remained higher among the older adults when compared to younger adults and stayed higher for a longer period of time leading to increased daytime drowsiness. The more melatonin the person took, the more pronounced these effects.

As a result, researchers recommend older adults start with the lowest dose of melatonin possible. Lower doses may help older adults sleep better without disrupting their circadian rhythms and causing prolonged drowsiness.

Older adults with dementia should avoid melatonin, according to the American Academy of Sleep Medicine.

How Much Should You Take

The majority of studies have researched doses ranging from 0.3 mg to 10 mg as the safest and most effective dose. However, the ideal dose depends on age, weight, the problem youre trying to address, and any current medications that youre taking. Generally speaking, the ideal dose is the lowest dose possible that provides the desired effects. Most experts recommended starting at the lowest dose and slowly increasing until you get the desired effect.

If youre unsure which dose to start with, its best to talk to your doctor.

Melatonin Supplements Arent Subject To Safety Standards

Because melatonin is licensed as a natural health product, companies are not subject to the same manufacturing scrutiny when making melatonin supplements as they are when producing pharmaceuticals. That means you dont have any reassurance that the package of 3 mg melatonin pills you picked up actually contains 3 mg doses. A study published in the Journal of Clinical Sleep Medicine in January looked at 31 products and found that 71 percent of them actually contained doses that were more than 10 percent off what was stated, and 26 percent contained unlabelled serotonin, which is not authorized for sale as a supplement.

When Should Melatonin Be Taken

The best time to take melatonin is 90 to 120 minutes before you plan on going to bed. This allows the body to metabolize the melatonin and eventually set off the OK-were-now-going-to-sleep signals in your brain. Melatonin for kids or adults shouldnt be administered as a direct sleep aid right at bedtime, which is one of the biggest misconceptions surrounding melatonin use.

If somebody with sleep difficulties is reporting that theyre sleeping better when taking melatonin right before bed, thats probably not whats getting them to sleep, since a two-hour window is generally needed for melatonin to work effectively. What may be happening is a type of placebo effect, where the thought of a sleep aid alone can feel really good and have a reinforcing effect. It can make the person feel calm, which may help them fall asleep.

What Are Melatonins Side Effects On Kids

Potential, though mild, side effects of melatonin use in children include agitation, headache, dizziness and; drowsiness.; While it is not known for sure and more research on the long term use of melatonin in children is needed, its important to remember that melatonin is a hormone, and, as such, there is a possibility that it may affect hormonal development, including puberty and menstrual cycles.2

The Bottom Line

If your child occasionally has trouble falling asleep, instilling better sleep habits and routines might be in order. But if your child experiences significant difficulty falling asleep, melatonin might benefit, especially when its used in combination with behavioral interventions and healthy sleep practices.5

Most importantly, talk with your childs pediatrician before giving them melatonin to make sure its right for them, to understand the proper dosage, and to rule out any underlying health issues.
