How Dermatologists Treat Severe Acne

How to Treat Cystic & Hormonal Acne | Ask the Skin Care Experts

When a patient has severe acne, a dermatologist often recommends treating it with one of the following.

Antibiotic + medicine you apply to the acne: This is often the first treatment recommended for severe acne. Taking an antibiotic can reduce the redness and swelling of acne. The medicine you apply to your skin works on reducing bacteria and clogged pores.

If this treatment fails to clear your skin, your dermatologist may switch your antibiotic or talk with you about isotretinoin .

Isotretinoin: This is a potent medicine that attacks all four causes of acnebacteria, clogged pores, excess oil, and inflammation . About 85% of patients see permanent clearing after one course of isotretinoin.

Due to possible side effects, you will need to carefully consider whether you want to take this medicine. If you decide to take isotretinoin, you must enroll in a monitoring program.

Birth control pill : A birth control pill can be an effective part of an acne treatment plan. When treating severe acne, a birth control pill may be used along with an antibiotic or a medicine called spironolactone to get the acne under control.

Give treatment time

Give your acne treatment time to work. It can take two to three months to see improvement.

If the pill may be an option for you, its important to tell your dermatologist about all of your medical conditions, including heart disease. Your dermatologist must weigh the risks of using a birth control pill to treat acne against the benefits.

    How To Treat Cystic Acne

Acne is a skin condition that affects the face, chest, back, shoulders, and upper arms. People who have cystic acne may feel embarrassed and want to know how to get rid of it. The good news is that acne can usually be cleared up with proper care.

Overall, there are four types of acne: comedonal , inflammatory , nodular and cystic . Cystic acne is the most severe form of acne. It can be deep and painful and take weeks or months to heal.

The main causes of cystic acne are hormonal imbalance and genetic predisposition. In most cases the cysts are triggered by stress and diet is a possible factor. For some people, cystic acne causes pain and psychological distress even self-rejection during the period when they are most visible.

In order to treat it, there are several methods, but before you start using any of them, it is important to consult with your doctor. The doctor will inspect your cystic acne and determine the best treatment method, taking into consideration factors such as your age and heredity.

    Treatment methods may include:

    – Topical or oral contraceptives

    – Isotretinoin for severe cystic acne

Once the doctor prescribes a treatment method, it is very important to use it properly in order to eliminate cystic acne. As a rule, you can apply home remedies after washing your face with water and cleansing it with a mild soap.

    • Don’t touch it! Your hands have a lot of bacteria that can further clog your pores or even lead to infection. Leave it alone, and give it time to heal.
    • Ditch the product. Unless it’s recommended or approved by your physician, try to avoid using cleansers and creams. Some of these aren’t what they seem, and can sometimes worsen the problem.

    Why Is Hormonal Acne Important

Hormonal acne does not always respond fully to treatment with acne creams, such as topical retinoids, and antibiotics. In some cases hormonal acne does not even respond well to treatment with Isotretinoin . Hormonal acne is more likely to come back after a course of Roaccutane has successfully cleared it.

Besides being stubborn to treat, hormonal acne causes redness for prolonged periods, scarring and pigmentation. Some types of hormonal acne cause a large number of comedones to develop especially on the sides of the face temples, cheeks and jaw line.

Enlarged pores on the nose and cheeks are commonly seen with hormonal acne. Hormonal acne can be really frustrating and cause a reduced quality of life for sufferers.

Hormonal acne can be distinguished from fungal acne on the type of spots and their location. Fungal acne causes smaller spots that are more evenly sized and itchy.

Finish With A Medicated Spot Treatment To Kill The Bacteria

Benzoyl peroxide works by lowering levels of acne-causing bacteria on the skin, thereby reducing inflammation, which is why Zeichner recommends using the ingredient as a spot treatment. Skin-care products come in 2.5, 5, and up to 10 percent concentrations of benzoyl peroxide, but you don’t necessarily have to max out on it. “Studies have shown that even low concentrations of benzoyl peroxide are as effective as higher concentrations, but are associated with less dryness of the skin,” he says.

Zeichner recommends looking for leave-on acne spot treatments with 2.5 percent benzoyl peroxide, such as Neutrogena Rapid Clear Stubborn Acne Daily Leave-On Mask or Paula’s Choice Clear Daily Skin Clearing Treatment.

How I Cured My Adult Hormonal Cystic Acne Naturally (no accutane)

Cystic acne is a tough skin problem that I treat with multiple medicines and acne fighting ingredients. In mild cases of cystic acne topical acne products may be sufficient to control acne breakouts.

In my practice, everyone who has acne is on some sort of retinoid. Retinoids can be prescription strength, or they can come in a milder form called retinol. Retinoids are family of vitamin A. Retinoids help to keep pores unclogged, they fight pigment problems and help soften the appearance of acne scars. Retinol has age fighting benefits which means it is an ingredient that I recommend throughout life .

I also use benzoyl peroxide, which is a powerful acne-fighting ingredient for cystic acne. Benzoyl peroxide is the best topical medicine to kill the acne causing bacteria called P. acnes/C. acnes. It also helps unclog pores. Benzoyl peroxide can be used as a cleanser or as a leave-on topical product.

A great benzoyl peroxide cleanser for people with acne is Foaming Benzoyl Peroxide Acne Treatment Cleanser. It’s an easy product that cleans skin and medicates with a full 10% benzoyl peroxide to fight acne all in one. I recommend using the benzoyl peroxide in the morning and waiting until your evening routine to use a non-benzoyl peroxide cleanser at night with the Retinol Cream. This is because benzoyl peroxide and retinol work best when they are not directly combined.

Insulin And Insulin Growth Factor 1

Insulin stimulates the growth and maturation of sebaceous glands. This action is mediated through upregulation of GH receptors on the sebocytes by insulin. Moreover, insulin inhibits SHBG production from the liver and further plays a positive feedback effect on adrenal and ovarian androgenesis. The controversial link between diet and acne can be explained by the fact that highly glycemic index foods result in insulin release and in turn excess androgen and sebum production.

    Other Reasons For Elevated Testosterone:

DHEA-S is another hormone that acts like a male sex hormone in the body and which stimulates oil production. It is different from testosterone however because it is not produced in the ovaries. It is produced by the adrenal glands.

When you are stressed out, DHEA-S levels rise.

This is a problem for many women with PCOS, since it exacerbates symptoms like cystic acne. It is commonly a problem for women with hypothalamic amenorrhea, too, since women with HA have already put a lot of stress on their bodies.

This is an even greater problem for women who may have both hypothalamic amenorrhea and PCOS .

Then there is one more hormone that causes acne, though not as much as testosterone and DHEA-S. Its progesterone.

Progesterone does not increase oil production in the skin, so it is not quite as cystic as testosterone and DHEA-S are. But it does block estrogen activity in the skin. Estrogen is protective to the skin, so many women who have high progesterone levels often because of the pill or the progesterone IUD suffer from increased acne. You can read more about birth control options and how to manage their health effects in this PDF I wrote on birth control, here.

Yet if testosterone levels or DHEA-S levels are so high, not even high estrogen will be enough to curb their effects.

    What Is Hormonal Acne

âHormonal changes in the body typically play some role in stimulating the oil-producing structures of the skin that lead to acne â all acne has some hormonal component,â says Dr. Carlos Charles, board-certified dermatologist and co-founder of 4.5.6 Skin. However, there are specific hormones that can increase production of oil in the skin.

For instance, itâs common to experience hormonal acne during your menstrual period. âWe have higher levels of androgens and estrogens during the follicular phase and periovulation causes increased sebum production, resulting in higher levels of skin lipids and subsequent increase in skin microflora that leads to breakouts,â says Dr. Loretta Ciraldo MD FAAD, a Miami-based board certified derm and founder of Dr. Loretta Skincare.

You might notice some bleeding, scarring, pain, and hyperpigmentation when it comes to these types of breakouts. âBecause of the deep inflammation associated with these pimples, in melanin-rich skin they can often leave stubborn hyperpigmentation once they resolve, or reddish blotches that may take weeks to resolve in lighter skin tones,â says Dr. Charles.
