I have not met a parent who is eager to medicate their child. Such decisions are made with a lot of soul-searching, and frequently after unsuccessful attempts to address sleep problems via behavioral changes. Treatment options are limited. There are no FDA-approved insomnia medications for children except for chloral hydrate which is no longer available. Personally, I use melatonin for children commonly in my practice. It is very helpful for some children and families. I appreciate Dr. Kennaways concerns but I have seen first hand the consequences of poor sleep on children and families. I always investigate to make sure that I am not missing other causes of insomnia . My end goal is always to help a child sleep with a minimum of medications. I know that this is the goal of parents as well. Some children, especially those with autism of developmental issues, will not be able to sleep without medication. So, melatonin may be a good option for your child if:

  • Behavioral changes alone have been ineffective
  • Other medical causes of insomnia have been ruled out
  • Your physician thinks that melatonin is a safe option for your child and is willing to follow his or her insomnia over time

Is it okay to give my teenage daughter melatonin to help her sleep better? I have read that it’s a safe alternative to sleep medicines.

Answer:

Although melatonin has been shown to be useful for treating sleep problems in adults, it has not been carefully studied in children. Due to the lack of scientific evidence, and because of some potentially harmful side effects, melatonin is not recommended as a sleep aid for children and teens. Making lifestyle changes that can enhance healthy sleep is a better alternative for most people your daughter’s age.

Melatonin is a hormone produced by your brain’s pineal gland. Melatonin appears to be linked to the body’s sleep/wake cycle, or circadian rhythm. The release of melatonin is stimulated by darkness and suppressed by light. The levels of melatonin in your blood are highest just before you go to sleep.

Some research suggests that, in adults, melatonin supplements might be helpful in treating jet lag or reducing the time it takes to fall asleep. The most common side effects from taking melatonin are daytime sleepiness, dizziness and headaches. Less common side effects can include abdominal pain, mild anxiety, irritability, confusion and feelings of depression. It is not known how common or severe these side effects might be in children.

Teach Your Child Healthy Bedtime Habits

Whether or not you choose to give your child melatonin as a sleep aid on a limited basis, having a regular bedtime routine should make it easier for your child to fall asleep.

Try these ideas:

  • establish a set bedtime, so your child expects to go to sleep at the same time every night
  • have a set wakeup time each morning, so your child is tired at bedtime
  • put away smartphones and tablets an hour before bed, and keep devices out of the bedroom
  • create a relaxing bedtime routine bathing, reading and listening to music are good options
  • keep your childs bedroom cool and dark
  • expose your child to bright light in the morning

If your child often has trouble falling asleep, talk to the pediatrician about possible causes of the problem, rather than giving your child melatonin gummies regularly.

Melatonin Supplements Arent Subject To Safety Standards

Because melatonin is licensed as a natural health product, companies are not subject to the same manufacturing scrutiny when making melatonin supplements as they are when producing pharmaceuticals. That means you dont have any reassurance that the package of 3 mg melatonin pills you picked up actually contains 3 mg doses. A study published in the Journal of Clinical Sleep Medicine in January looked at 31 products and found that 71 percent of them actually contained doses that were more than 10 percent off what was stated, and 26 percent contained unlabelled serotonin, which is not authorized for sale as a supplement.

Its hard to know for sure. An article the New York Times, Parents Are Relying on Melatonin to Help Their Kids Sleep. Should They?, noted that melatonin sales overall had increased by 87% in the year prior to March 2020. The Times conducted a survey of 933 parents with children under age 18. One third had a history of sleep difficulties in the past year. Over half the parents reported giving melatonin to their children at one time.

How Much Melatonin Should I Take

Its best to start with the lowest recommended dosage of melatonin for your age. From there, you can gradually increase your dosage until you find a dose that helps you fall asleep without causing any side effects. A safe starting dose for adults is between 0.5 milligram and 5 milligrams of melatonin. Older adults may find lower doses, starting with 0.1 milligram, to be safe and effective. Children should not take melatonin unless recommended by a doctor.

Over-the-counter melatonin may come in standard amounts like 1 milligram, 3 milligrams, or 5 milligrams. You can use a pill-cutter to cut the tablets in half or quarters to create a smaller starting dose.

In addition to giving the right amount of melatonin, its equally important to give it at the right time. If your toddler has a hard time falling asleep, experts recommend giving them their dose about 1-2 hours before bedtime. But some kids need help with the sleep/wake cycle throughout the night. In these cases, pediatric sleep expert Dr. Craig Canapari suggests a low dose at dinnertime. It can really depend on why your toddler needs melatonin, so definitely talk to your pediatrician about the right time to administer it, too.

This last quote is probably closest to where we actuallyare. However, it is clear there has been a dramatic increase in use ofmelatonin in children in the past five years or so. For example, in England, estimates have found a 25percent increase in melatonin prescriptions in children under 18 between2015-16 and 2017-18, and a 40 percent increase in the extended release formsduring the same period. Is melatonin safe for babies?

Melatonin can be a tricky medication to dose. Effects change depending on when you give it compared to your childs usual sleep schedule. Thus, a small dose a few hours before bedtime can have more of an effect than a large dose given at bedtime. In some situations dosing may the opposite effect. This is a special case and should be addressed with your physician. A couple of rules of thumb.

  • Timing: For shifting sleep schedules earlier 36 hours before current sleep onset is best. For the sleep onset effects, 30 minutes before bedtime is recommended. Remember, not every child gets sleepy with melatonin.
  • Dosing: In general, I would start at a low dose and increase slowly. Recognize that melatonin, unlike other medications, is a hormone, and that lower doses are sometimes more effective than higher ones, especially if the benefit of it reduces with time.
  • Good Sleep Hygiene is Critical: Melatonin is not a substitute for good sleep hygiene practices and should only be used in concert with a high quality bedtime, limitation on light exposure, and an appropriate sleep schedule.
  • When possible, purchasing a USP Verified preparation;may indicate that the product is manufactured to the;requirements of the U.S. Pharmacopeial Convention, which could mean that the quality controllers are tighter.

Melatonin Could Help Kids With Adhd And Autism

Kids with attention disorders and autism commonly develop sleep problems that can be helped with melatonin. Additionally, medication used to treat ADHD can lead to difficulty falling asleep. That was the case for Jackie Botman,* whose son has ADHD. The eight-year-olds medication was keeping him up until 9:30 or laterhe was wired and couldnt settle down. He was resistant to calming down and having quiet time. He became chronically tired, says Botman, who lives in Toronto. Now her son takes a ;5 mg dose of melatonin when hes brushing his teeth, then gets into bed and falls asleep in a reasonable amount of time.

Although Cummings agrees that melatonin can help kids with ADHD or autism, he says the downside is that theres a high relapse rate: Once you take your kid off melatonin, they are almost guaranteed to go back to having difficulty sleeping. It sets up a situation where those kids, to function well in life and in school, may need to use the melatonin on a longer-term basis, says Cummings.

Chronic Sleep Onset Insomnia And Melatonin:

Problems with falling asleep are common in children, just like in adults. In children with chronic difficulty falling asleep within 30 minutes of an age-appropriate bedtime. Use of melatonin results in less difficulty with falling asleep, earlier time of sleep onset, and more sleep at night. The initial studies used pretty high doses, but later studies comparing different doses showed that dose didnt matter, and that the lowest dose studied was as effective as the highest. This is likely due to the fact that ALL these doses were well above the amount produced naturally in the children. Timing between 67 PM was more effective than later doses. The authors point out that a midafternoon dose would have the best effect but that afternoon dosing would have the unpleasant side effective of making children sleepy in the afternoon. .

Do Kids Ever Need Sleeping Pills

In generations past, parents often gave small children a little something to put them to sleepfrom a few drops of opium to a nip of brandy. These days, parents are more likely to ask for a prescription.

However, when it comes to giving medicine to young children, the best rule of thumb is less is better. This is especially true for sleep medications. How Much Melatonin Can A Child Take

I usually advise people to start at low doses of 0.5 milligrams or 1 milligrams and give it about 90 minutes before bedtime, DeBlasio says. Levinson says that the general recommended dosage utilized in studies of children is between 0.5-6 milligrams nightly. Again, though, melatonin isnt regulated by the FDA, which can make dosage tricky. Research has shown that the stated amount of melatonin in a supplement can be off by over 400%, and other compounds, such as serotonin, can be included, Levinson says. The dosage variability can even be very high when the same brand is purchased on separate occasions. Families should always check with their childs pediatrician before giving melatonin to their child.
