What Is Saliva Testing For Hormones

Don't Miss

Timing Your Saliva Collection For Accurate Test Results Is Important Read The Instruction Booklet

What is a Saliva Hormone Testing?

1. Hormones are highest in the morning so testing first thing is important.

2. There are certain days when testing is optimal.

MEN can test anytime.

WOMEN who do not have a menstrual cycle can test anytime.

WOMEN who are menstruating need to test between days 19 and 21 with day 1 being the first day of the previous menstrual cycle.

Symptoms Of Hormonal Imbalance To Look Out For

Normal changes in hormones will have almost no effect on your body. However, underlying medical conditions that cause hormone imbalance show visible symptoms. If the change in hormones is due to improperly functioning endocrine glands, here are a few symptoms you should look out for:

  • Sudden Gain or Loss in Bodyweight
  • Sweating Without Any Reason
  • Infertility
  • Erectile Dysfunction

If you are experiencing any one of the above symptoms, youcould have hormonal imbalance issues due to a medical condition.

My Patient Is Taking The Birth Control Pill Will This Interfere With Hormone Testing

Oral contraceptives and other hormonal forms of birth control do not directly interfere with saliva hormone testing or cause falsely elevated values. These contraceptive medications have the effect of suppressing the bodys own natural production of estrogens, progesterone, testosterone, and the pituitary hormones FSH and LH. Thus, when a patient is using a hormonal contraceptive, we typically find low levels of these endogenous hormones on testing. We generally do not recommend testing with the Cycling Female Hormone Panel when a patient is using a hormonal contraceptive. However, the Adrenal Stress Index panel may still provide useful information when a patient is on a birth control medication. Although levels of DHEA and 17-hydroxyprogesterone may be suppressed in some patients, cortisol levels should not be significantly altered by the use of contraceptive medications.

Serum Or Blood Testing

Most serum hormone tests define the normal range of hormones very broadly, which is a distinct disadvantage to their use. After a womans blood has been drawn, a portion of the blood sample is used to measure hormone levels. Most serum testing measures the level of free hormone , the level of the total hormone , or a calculated combination of both free and total levels of hormone. It is not an accurate reflection of the bioavailable hormone . In addition, the results of the serum hormone tests are often inconsistent, especially if the hormone value indicated is in the low-normal range. Serum testing is less helpful in monitoring women using transdermal forms of hormones, as the hormone is bound to the red blood cell surface and does not register in the serum in very high amounts.

Many women whose serum hormone test results are normal cannot understand why they continue to experience the symptoms of hormone imbalance.

Understanding Different Types Of Hormone Tests

Saliva Test Kit (3 Hormone Test)

Hormone imbalances and fluctuations can leave you feeling awful. Identifying these conditions via hormone testing is essential to re-establish a healthy balance.

Hormone testing allows you to better understand how your body is operating under the surface and helps unravel the complicated internal reality that symptoms alone cant reliably reveal. If initial testing reveals an imbalance or problematic fluctuations, your hormone values can be used to create a treatment plan customized to your needs. Ongoing testing will be critical to monitoring the efficacy of treatment and fine-tune your dosage to achieve optimal hormone levels. Your hormone health practitioner knows best which types of hormone tests you need, but understanding your options can help you understand what to expect.

Is Saliva Testing For Hormone Imbalance Reliable

When you do a hormone test to understand if you have any hormonal imbalance that can be caused by a medical condition, you will want to make sure that the test youre using is reliable, and gives you correct values that reflect the actual hormone levels in your blood.

There are two forms of hormones in your blood, free and protein bound. Free hormones are also known as unbound hormones and they are biologically active, whereas, protein bound hormones are non-bioavailable and have no function on other cells.

Whereas testing hormones in your blood serum can measure both free and bound proteins, and therefore give you more information, saliva testing only measures the bioavailable or unbound hormone, which is the portion that is delivered to the tissues in the body. This means that the hormone levels found in your saliva are much lower than how they actually are found in your blood.

Also, the hormone levels in your saliva change depending on the time of the day, your diet and hydration, as well as how much saliva you have in your mouth. All of these factors can change the hormone levels that are measured, which is not good if you are looking for a precise, reliable test for your hormones.

Saliva testing is currently not considered an accurate method to test your hormone levels and find out if you have a hormonal imbalance.

    Uses In Behavioral Research

Saliva testing also has specific uses in clinical and experimental psychological settings. Due to its ability to provide insight into human behavior, emotions, and development, it has been used to investigate psychological phenomenon such as anxiety, depression, PTSD, and other behavioral disorders. Its primary purpose is to test cortisol and alpha amylase levels, which are indicative of stress levels. Salivary cortisol is a good stress index, with increased levels of cortisol correlating positively with increased levels of stress. Cortisol levels rise slowly over time and take a while to return to base level, indicating that cortisol is more associated with chronic stress levels. Alpha amylase, on the other hand, spikes quickly when confronted with a stressor and returns to baseline soon after the stress has passed, making salivary amylase measurement a powerful tool for psychological research studying acute stress responses. Samples are usually collected from participants by having them drool through a straw into a collection tube while experiencing a stimulus, with samples taken every few minutes to record the gradual change in stress hormone levels. Because the collection of saliva samples is non-invasive, it has the advantage of not introducing further stress on the participant that may otherwise distort results.

Salivary Test For Bioidentical Hormone Therapy

5 Reasons We Prefer Saliva Testing VS Standard Blood ...

The American Association of Clinical Endocrinologists Reproductive Medicine Committees position statement on bioidentical hormones noted that Salivary hormone level testing is recommended by many BH proponents as a way of providing patients with individualized therapy. Such tests are available to consumers over the Internet. Some of the websites include elaborate questionnaires supposedly designed to establish the type of saliva testing needed. The results of these tests are subsequently used to determine the type and dosage of compounded formulations. Only a few types of salivary hormone testing methods are FDA/CLIA approved. In fact, the vast majority of the salivary hormone tests results contain the disclaimer that those tests are not FDA/CLIA approved and should be used only for research purposes. Yet such tests are still utilized to support clinical decisions by some supporters of BH . the limited research, although interesting, does not prove that salivary testing can be used as reliable ancillary tests for clinical purposes . the evidence often quoted by Salivary Test promoters simply do not pass the muster of the level 1 or even 2 of the Level of Evidences as endorsed by AACE .

Salivary Testing Of Biomarkers For The Diagnosis Of Cancers

    Breast Cancer

Porto-Mascarenhas and colleagues assessed the capability of salivary biomarkers in the diagnosis and monitoring of breast cancer . Studies were eligible for inclusion if they evaluated the potential diagnostic value or other discriminatory properties of biomarkers in saliva of patients with BC. The search was performed in 6 electronic databases . In addition, the biomarkers were classified according to their potential clinical application. These researchers identified 567 pertinent studies, of which 13 met the inclusion criteria. Combined biomarker approaches demonstrated better ability to predict BC patients than individual biomarkers. As single biomarker, namely proline, reported great capacity in both early and late stage BC diagnosis taurine showed interesting capability to identify early BC individuals. Furthermore, valine also demonstrated excellent diagnostic test accuracy for advanced stages of BC. Only 7 studies reported sensitivity and specificity, which varied considerably from 50 % to 100 %, and from 51 % to 97 %, respectively. In general, salivary biomarkers identified advanced stages BC better than early stages. The authors concluded that there is currently limited evidence to confirm the putative implementation of salivary biomarkers as diagnostic tools for BC however, this review provided new research directions.

My Patient Is Using Fluticasone Nasal Spray Should This Be Stopped For A Period Of Time Before Testing

For safety reasons, we advise practitioners to follow proper protocols for discontinuing use of steroid medications. When medically appropriate, fluticasone and other steroid medications may be stopped to allow for more accurate hormone testing. All medications containing cortisone, hydrocortisone, or any other corticosteroid including over-the-counter and prescription skin creams and ointments, hemorrhoid creams, steroid eye drops, steroid nasal sprays , steroid inhalers, and oral corticosteroid medications can potentially interfere with cortisol testing and should be avoided for 24 hours before testing and on the day of testing. In addition, as noted above for prednisone, all corticosteroid medications may potentially suppress normal adrenal hormone levels. As such, practitioners may decide to wait several days to several weeks after a patient has discontinued using these steroid medications, depending on length of use, in order to more accurately assess baseline hormone levels.

Differences Between Analyzing Serum And Saliva

It is often difficult for healthcare providers to assess accurate hormone levels with serum, which makes it even more difficult to design a hormone replacement therapy for patients. Serum contains both bound and unbound hormones, yet a reliable method to identify an accurate ratio between these different hormones has yet to be determined.

Saliva, however, provides an accurate level of unbound hormones available in circulation. This is commonly a much lower value in comparison to serum, as it only assesses free hormones. When clinicians utilize saliva to measure effectiveness, they often find that it is higher than serum, indicating a decrease in hormone replacement therapy is necessary. By strictly using serum, clinicians have found that they needed to give excessive amounts of hormone therapy in order to cause an increase in serum hormone levels.

A Better Choice For Clinicians And Patients

Unlike blood-based tests, saliva diagnostics can measure the amount of free, unbound hormone that is available to act on a target tissue. About 95-99% of steroid hormones circulating in the bloodstream are bound to carrier proteins. Saliva contains unbound, bioavailable hormones. Therefore, salivary concentrations better represent circulating levels of free hormones, 1 which are likely to affect the body and be responsible for symptoms related to excessive or deficient hormone levels.

Among the advantages of saliva tests versus blood tests are the non-invasive and stress-free nature of saliva sampling. Hormone testing may often require baseline measurements as well as periodic tests to monitor a particular condition or adjust therapeutic dosing. Saliva sampling must be done correctly, but today’s collection devices make it easy, and patients can collect their own samples during the day or night as needed, to measure fluctuations in hormones such as cortisol or melatonin, for example. Saliva testing also provides an easy and convenient way to acquire the periodic measurements of various sex hormones in women of reproductive age for the treatment of fertility problems, or in peri- or post-menopausal women to adjust and personalize dosing in hormone replacement therapy.
