How Does Melatonin Work
Before you consider bedtime supplements as a parent, you should understand sleep cycles. Here’s how they work: Our brain starts releasing melatonina hormone produced in the pineal glandas it gets dark at night to help us fall asleep. In the morning, when we need to wake up, the melatonin mostly shuts off. Essentially, melatonin works by regulating your circadian rhythm, also known as your “internal clock.”
Supplements are a synthetic form of natural melatonin, and they usually make kids tired within 30 minutes. They’re available over-the-counter as gummies, pills, liquids, or chewable tablets. Research shows that melatonin supplements can help children who have autism, ADHD, and other neurodevelopmental disorders fall asleep faster, and a few studies have shown similar effects in typically developing kids.
Melatonin Dosage For Older Adults
Our melatonin levels naturally , disrupting the sleep-wake cycles for many older adults. As a result, older adults may have an increased sensitivity to melatonin. In a meta-analysis of 16 studies, melatonin dosages between 0.1 milligram and 50 milligrams per kilogram were administered to older adults aged 55 to 77 years old. In all of the studies, the melatonin levels remained higher among the older adults when compared to younger adults and stayed higher for a longer period of time leading to increased daytime drowsiness. The more melatonin the person took, the more pronounced these effects.
As a result, researchers recommend older adults start with the lowest dose of melatonin possible. Lower doses may help older adults sleep better without disrupting their circadian rhythms and causing prolonged drowsiness.
Older adults with dementia should avoid melatonin, according to the American Academy of Sleep Medicine.
When To Talk To Your Doctor About Melatonin
Sleep is essential for children for a number of reasons. During sleep, the body releases growth hormones and repairs damaged tissue and muscle. Appropriate sleep in children is linked to positive school performance.
If your child is struggling to fall asleep or get sufficient sleep, talk to your healthcare provider. They can help identify the cause of sleeplessness. Common reasons for a lack of sleep in children include insomnia and delayed sleep-phase syndrome.

Higher doses of melatonin tend to be reserved for children with medically related sleep issues, such as blindness, autism, or ADHD, Dr. Karp notes. He also points out that like many medications, melatonin has possible side effects including drowsiness, headache, dizziness, increased urination, bedwetting, and agitation.

How Much Melatonin Can I Give My Child
Melatonin usually comes in 3 mg or 5 mg tablets. Five milligrams is considered a relatively high dose for a young person most children take a 3 mg or even a 1 mg dose.
In a study on melatonin for kids with ADHD, sleep onset advanced by 27 to 48 minutes with melatonin, and sleep duration increased by as much as 62 minutes.7 The children, between 6 and 12 years old, were given either a 3 mg or 6 mg dose of melatonin depending on their weight.
But When Should Your Child Take Melatonin
To help your child go to sleep, it’s usually recommended that they take it around 30-60 minutes before you want them to go to bed. But you can also try giving it at different times to see when works best for your child.
Some studies have showed promise for the use of melatonin in shortening the time it takes for children to fall asleep and reducing the number of awakenings, but not necessarily shortening total sleep time. And other studies have shown no benefit to melatonin usage at all.
But keep in mind that there is still a need for large scale studies that will demonstrate whether or not melatonin is effective or sale for some young insomnia patients.
It may be true that melatonin is effective and safe for some types of insomnia and for children but not for other types of sleep problems. How much to take, when to take it and its effectiveness, if any, for particular disorders is only beginning of what we need to understand.

Is Melatonin Safe For My Child
Does a big school presentation have your little one up again? Are they having a hard time counting sheep? If your child is having trouble falling and staying asleep, they arent alone.
According to the American Academy of Pediatrics, 15 to 25% of children and adolescents have trouble with sleep. It may not only impact your sleep but it can also lead to some difficult behavioral and health problems in your child.
If youve tried everything in the book, from limited technology use before bed to regularly scheduled bedtimes and calming sleep routines to no-avail, you may wonder if melatonin is the answer.
We spoke with Bryan Kuhn, a clinical pharmacist and poison information specialist with Banner Poison & Drug Information Center, to discuss the supplements risks and benefits.
Should Parents Give Their Kids Melatonin
First and foremost, consult your pediatrician about using melatonin. She can recommend a trusted formula, the right dosage for your child, and when and how often to use. A pediatrician can also help treat underlying disorders that may be related to lost sleep or offer recommendations for various sleep solutions.
Dr. Karp also advises parents to avoid melatonin until all other options are exhausted. Its not something you want to use after one bad night, for example. If your tyke is struggling with sleep, Id recommend trying other methods , before turning to a supplement, says Dr. Karp.

First Off When Should You Use Melatonin With Your Toddler
This is where parents get a little confused. If your child can fall asleep on their own about 30 minutes after you put them to bed, melatonin may not be necessary! The natural sleep aid can be very helpful, however, if your child has a sleep dysfunction. For instance, if they cant fall asleep and lie awake for hours, or fall asleep and then wake several times during the night.
It can also be very helpful for kids on the autism spectrum, or those whove been diagnosed with ADHD. Kids with these disorders are well-known to have a lot of trouble falling asleep, and studies have shown melatonin to be effective in shortening the time it takes them to fall asleep.
It Sounds Great Why Should I Worry About Melatonin
There are several areas for concern, specifically known and theoretical side effects, and problems with preparations.
This means there is substantially less regulatory oversight in terms of safety and efficacy. I also find that the labelling of preparations is frequently misleading. Take the example of this liquid preparation, which many of my patients have tried. It is labeled as 1 mg but each dropperful contains 0.25 mg.

What may be happening is a type of placebo effect, where the thought of a sleep aid alone can feel really good and have a reinforcing effect. Most of us have seen melatonin suppliments in health food stores or even in your average grocer chain. The interesting fact is that no other hormone is available in the United States without a prescription, other than melatonin. That’s because melatonin is contained naturally in some foods, which explians why the U.S. Dietary Supplement Health and Education Act of 1994 allows it to be sold as a dietary supplement. Dietary supplements do not need to be approved by the Food and Drug Administration or controlled in the same way as drugs.
Is Melatonin Safe Forchildren
Its no wonder parents are uncertain about this. If you surfthe web, youre likely to get mixed messages:

This last quote is probably closest to where we actuallyare. However, it is clear there has been a dramatic increase in use ofmelatonin in children in the past five years or so. For example, in England, estimates have found a 25percent increase in melatonin prescriptions in children under 18 between2015-16 and 2017-18, and a 40 percent increase in the extended release formsduring the same period. There are no similar statistics in the U.S. forover-the-counter melatonin, but it is now rare in our experience that childrencoming to the Boston Childrens Sleep Clinic with insomnia are not already onmelatonin, usually recommended by their pediatrician.

So How Much Melatonin Should My Child Take And What Are The Possible Side Effects
Because it is not categorized as a drug, synthetic melatonin is made in factories that are not regulated by the FDA. This can be a concern, especially since the listed doses may not be controlled or accurate, meaning the amount of melatonin in a pill you take may not be the amount listed on the package.
Most commercial products are offered at dosages that cause melatonin levels in the blood to rise to much higher levels than are naturally produced in the body. Taking a typical dose may elevate your blood melatonin levels to 1 to 20 times normal.
For melatonin to be helpful, the correct dosage, method and time of day it is taken must be appropriate to the sleep problem. Taking it at the “wrong” time of day may reset your biological clock in an undesirable direction. While there are real concerns about the widespread use of melatonin sold as a consumer product, there have not been any reported cases of proven toxicity or overdose.
That’s why it is crucial to consult a pediatrician or sleep physician before administering melatonin for your child before bed.

How Melatonin Helps Babies Sleep
For babies who have trouble sleeping, it is sometimes determined that their natural melatonin levels are lower than they should be. Of course, this poses a problem when it is time to sleep, and it will need to be addressed in some way. If you choose to go the route of a melatonin supplement, you will find that the supplement encourages normal sleep cycles by normalizing the melatonin levels in your babys body.
Eventually, it is predicted that the melatonin supplement will help your babys natural sleep cycles regulate. In the meantime, melatonin for babies should be given about 30 minutes before its time to sleep.
Delayed Sleep Phase Syndrome And Melatonin
Delayed sleep phase syndrome is a common disorder in teens, where their natural sleep period is shifted significantly later than the schedule which their commitments mandates. Thus, teens with this disorder an unable to fall asleep by 12 AM in the morning or even later. I have seen kids who are routinely falling asleep between 45 AM. Melatonin has a clear role in this disorder, as small doses 34 hours earlier than sleep onset can be effective in managing this disorder. The reason for the delay is a marked delay in the DLMO, so melatonin dosing can move sleep periods earlier. For children with DSPS, giving a dose 46 hours prior to the current time of sleep onset, then moving it earlier every 45 days, is recommended, with low dose preparations. Are There Other Ways To Help Your Child Sleep Better
Melatonin has often been called the hormone of darkness because darkness triggers its release. It is suppressed by light, including the blue light emitted by digital screens. So one of the best ways to help a childs brain get sleepy is to dim the lights in your home and turn off those omnipresent screens an hour before bedtime.
Its also helpful to get exposure to bright light during the day, since sunlight helps regulate our sleeping patterns. When people are exposed to light in the morning, regardless of whether its sunlight or very bright artificial light, it helps nighttime melatonin production.
Try to keep a structured, consistent bedtime routine to preserve your childs sleep-wake schedule. For children 6 and under, difficulty sleeping is often a behavioral issue, experts said, and behavioral solutions might work.
To help your child fall asleep earlier, Dr. Canapari recommends making sure your child doesnt nap too late in the day. To get to sleep by 8 p.m., for example, make sure your childs nap ends before 4 p.m.
Also, match your childs bedtime to when they naturally fall asleep. Then try shifting bedtime earlier by 10 to 15 minute increments. Eventually, you will find a sweet spot, Dr. Owens said, that works for you and your child.
Dr. Canapari suggested thinking about ways to incentivize a childs time in bed and offering rewards as opposed to punishments.
