What Is A Hormonal Imbalance

Your body is comprised of various glands and tissues, all of which are part of your endocrine system. These glands deliver hormones throughout your body via your bloodstream. These are the chemicals that tell your organs what to do and when to do it.

Hormones are responsible for regulating some of your most critical bodily processes, including your:

  • Appetite
  • Sexual capacity
  • Internal temperature

A hormonal imbalance means that your body is producing too much or too little of a given hormone, such as insulin, estrogen, progesterone, or any other hormone. If your levels are off by even a little, it can create a significant change that you may feel to a severe degree.

How Do You Prepare For The Test

Thyroid Hormone Levels, Body Composition, Insulin ...

Since Cortisol levels vary at different times of the day, the timing at which blood sample is collected is important. A blood cortisol test is usually done in the morning when the levels are highest. Your doctor may, however, schedule your cortisol blood test at two different times in a day one early in the morning and one in the afternoon when the Cortisol levels are much lower.

A Cortisol Blood Test May Need The Following Preparations

  • Generally, fasting is not required for a cortisol blood test.
  • Since cortisol is released in response to stress, it is a usual advise by doctors to remain stress-free when reporting for the test.
  • You may be advised to withhold certain medications, health supplements, and herbal medicines before planning for the test.

Certain medicines may have to be stopped at least 24-48 hours before the test. Some medications which may alter blood cortisol levels and give incorrect results include

  • Synthetic glucocorticoids such as steroid drugs
  • Medicines containing estrogen such as oral contraceptives
  • Phenytoin

Other medications can interfere with thyroid laboratory measurements, but not with the actual functioning of the thyroid.

A few examples of these medications include:

Lastly, research has found that the supplement biotin can interfere with the measurement of thyroid blood tests. Therefore, it’s recommended that people who take biotin stop doing so two days prior to having their thyroid blood tests.

What About Fasting For A Blood Test During Pregnancy

There are several blood tests you may need if youre pregnant. These are designed to assess any potential health concerns that you or your baby might experience during pregnancy or after you give birth. Some of these tests will require you to fast beforehand. Your doctor will advise you how to prepare for each test.

Fasting is usually safe if youre pregnant, provided youre in good health and arent having a high-risk pregnancy. For your overall comfort, your doctor may advise you to drink extra water or to remain indoors, especially if the weather is very hot or humid.

Fasting may increase heartburn in some pregnant women. If you experience uncomfortable or concerning symptoms of any kind while youre waiting to have your blood drawn, let your doctor know immediately.

If youre seeing a doctor other than your obstetrician-gynecologist, make sure they are made aware of your pregnancy before your blood test.

What Do I Test If I Dont Have A Period

Do you have to Fasting for Thyroid Blood Tests? |Thyroid Series| Dr.Ravi Sankar | Hi9

If your period has stopped showing up , this is what is called amenorrhea.

  • Primary amenorrhea is when you havent gotten your period by age 15 or 16.
  • Secondary amenorrhea is when you have had a period, but now its gone.

If your regular period is missing for three or more months, or your irregular period is missing for six or more months, this is considered secondary amenorrhea.

Often, doctors will try to prescribe the birth control pill as a means of treating amenorrhea. If you begin birth control you will not be able to test FSH, LH, estradiol or progesterone accurately.

The Endocrine Society guidelines clearly advocate against the use of the pill in women with functional hypothalamic amenorrhea solely to restore a period or even improve bone mass. They also recommend educating patients who are currently using the pill that it may mask these issues.

What Happens If You Dont Fast Before Blood Test

Not fasting for a blood test will yield inaccurate results. As mentioned above, as the food is digested and metabolized, the nutrients which enter the bloodstream can interfere with test results.

If you happen to accidentally eat or drink something before a blood test, make sure to inform your clinician about it. In this case, both the clinician makes a note of it and test results are analyzed accordingly or you for better results, an appointment for the blood tests may need to be rescheduled.

Is Fasting Required For A Thyroid Test

Kidney Function Blood Test Do I Need To Fast

Most doctors will suggest you do not fast before your thyroid function test.

Research shows that fasting, especially early in the morning, may impact TSH levels. A fasting test typically results in higher TSH levels versus one done in the afternoon.

Because fasting causes higher TSH levels, it can make it hard to properly diagnose hypothyroidismwhich is diagnosed specifically by looking at TSH levels.

T3 Resin Uptake Results

A T3 resin uptake, also known as a T3RU, is a blood test that measures the binding capacity of a hormone called thyroxin-binding globulin . If your T3 level is elevated, your TBG binding capacity should be low.

Abnormally low levels of TBG often indicate a problem with the kidneys or with the body not getting enough protein. Abnormally high levels of TBG suggest high levels of estrogen in the body. High estrogen levels may be caused by pregnancy, eating estrogen-rich foods, obesity, or hormone replacement therapy.

Symptoms That Might Require A Fasting Lab Test

You may have nonspecific symptoms that are hard to diagnose. Or, you may have the following symptoms that could prompt your doctor to order a fasting lab test:

  • Excessive urination and thirst could indicate diabetes.
  • Swollen feet and ankles could indicate kidney disease.
  • A high blood pressure reading could indicate high cholesterol.
  • A yellowish tint to your skin and eyes may indicate a liver condition.
  • Fatigue and an irregular heartbeat could indicate an electrolyte imbalance.
  • Pale skin and leg cramps could indicate anemia or a vitamin B12 deficiency.

Many other symptoms could make you need a fasting lab test. Your doctor might order a fasting lab test to rule out some of these conditions.

When To Test Thyroid Labs

We cant discuss testing for hormone imbalances without looking at the thyroid. Thyroid testing is best down first thing in the morning and can be at any point in your cycle. We typically add these in when we are testing for other hormones.

Its important to stop any supplements with biotin at least 72 hours in advance as it can skew results.

At a minimum, I would suggest looking at:

Thyroid Blood Test Normal Range

Hormone
Anti-thyroglobulin Less than 20 IU/mL

Values above or below the reference range are considered abnormal. Hyperthyroidism and Hypothyroidism are the two commonest thyroid disorders.

If your tests results are abnormal, your clinician may want to evaluate further. He may advice you an ultrasound, CT scan, thyroid scan or a radioactive iodine uptake test. You may also need to undergo a few other blood tests to assess the causes of thyroid disorders.

Almost twenty years ago, the American College of Physicians, a pretty conservative organization, stunned the medical community by issuing a guideline that women over 50 should have a thyroid test at least once every five years. At that time, most physicians thought that they could diagnose thyroid problems themselves and that testing guidelines were unnecessary.

In fact, a couple of years ago, Time Magazine reported that hypothyroidism is one of the top 15 leading conditions doctors most often get wrong.

How To Treat Perimenopause When It Occurs

One of the biggest issues women face in perimenopause is the hot flashes. Low-dose oral contraceptives may be prescribed by a medical provider to reduce or eliminate this particularly bothersome symptom.

Activities which enhance a womans well-being are also strongly recommended by many medical providers. This means finding time to exercise on a regular basis, eating healthy foods, trying to get more sleep on a nightly basis, and to quit smoking if necessary.

Dryness during intercourse can also be bothersome for some women and their partner. Speak with your medical provider about what options may be available to you in this area.

Mood swings that are particularly bothersome can lead to future mental health concerns for some women. If you are concerned about this potential symptom of perimenopause, then talk to your doctor about how anti-depressants, counseling, and other options could be added to your treatment plan.

The goal of speaking to your medical provider should be to control bothersome symptoms so you can continue to chase your lifes goals. Use this guide to speak with your doctor about your concerns so that a treatment plan which addresses all of your needs can be established to make this happen.

Can You Drink Coffee If Youre Fasting Before A Blood Test

Nursing care to patients undergoing diagnostic procedures ...

Even if you drink it black, coffee can interfere with blood test results. Thats because it contains caffeine and soluble plant matter, which might skew your test results.

Coffee is also a diuretic, which means that it will increase how much you pee. This can have a dehydrating effect. The less hydrated you are, the harder it can be for the nurse or other medical professional whos doing your blood test to find a vein. This can make the blood test harder or more stressful for you.

Hormone Testing: What To Check And Why

Questions about hormone testing are common among my patients and on social media. Women want to know which hormones they need to check and why. They also want to know what hormones and other biomarkers can be tested at home. In this video and accompanying article, I cover the biomarkers that I check for myself and my patients approximately once per quarter. Note that not all biomarkers I discuss below have been validated for blood spot testing versus the gold standard of a blood draw for serum testing, which is what I use for treatment decisions. Still, one can infer trends and screen for problems to investigate further. My highest recommendation is to perform serum testing for all treatment decisions, though that may be difficult during quarantine and lockdown. I created this video to show the blood spot test that I am performing in lockdown, even with the caveat that it is not considered the gold standard for all tests mentioned in this article.
