I hated myself so much, whenever I would look in the mirror I would see an ugly disgusting slob. People would say I was a handsome young man, but I hated when they said that because, I was not a man, and I didnt see myself as handsome. Eight studies15 36 3941 44 46 51 used a follow-up design to assess changes in LBM seven studies assessed after 12 months,15 36 39 41 44 46 51 and one40 study reviewed patients who had been under treatment for 531 months. Seven of these studies,15 36 3941 44 51 including the large ENIGI study,15 and two studies40 51 with high post-GAHT testosterone , showed that total LBM was decreased by 3.0%5.4% following hormone transition . The one study that failed to demonstrate significant changes in LBM46 was not an outlier in any obvious way. The large ENIGI study15 was the only study in which the limits of agreement would indicate a change in LBM at the 95% CI. All studies reported an increase in total body fat mass in transwomen after hormone transition. Three cross-sectional studies41 51 54 compared transwomen with cisgender men. Two studies included hormone-naive transwomen.41 51 These studies reported 6.4% and 8.0% lower LBM than in cisgender men and reductions of 4% in LBM in the transwomen with 12 months of GAHT. The third cross-sectional study compared transwomen who had undergone at least 48 months of GAHT with cisgender men54 and reported 17% lower LBM in transwomen than in cisgender men.

Estrogen stops the pituitary gland from forming gonadotropin, which reduces androgen production. Androgens are a group of hormones that help with reproductive development, among other things. Reducing the number of androgens in the body helps a person transition by encouraging changes in fat distribution, breast development, and male pattern hair growth.
Doctors may also prescribe other anti-androgen drugs to help the transitioning process.
A person may not notice significant changes when starting estrogen hormone therapy. The timeline varies, and some people may see more significant changes than others.
People may notice more changes if they use anti-androgen therapy alongside estrogen therapy.

At What Point In My Gender Transition Can I Start Masculinizing Hormone Therapy
Together, you and your provider can decide the best time to start therapy. Some gender-nonconforming individuals go their entire lives without needing or wanting gender-affirming therapies or surgeries. Others begin therapy as soon as possible.
The Endocrine Society recommends that transgender adolescents start hormone therapy at 16 at the earliest. Earlier than 16, they recommend puberty blockers. The difference is that:
- Hormone therapy causes some changes to your body that are irreversible.
- Puberty blockers delay the onset of secondary sex characteristics associated with the gender you were assigned at birth. They prevent gender-nonconforming children from developing characteristics that may cause them to experience gender dysphoria, but they dont produce irreversible changes.
In some instances, transgender youths under 16 and their caregivers may decide that its best to begin masculinizing hormone therapy early for the well-being of the child. The right time to start treatment varies on a case-by-case basis.
