Mechanism Of Action Of Gnrh

After secretion of GnRH-secreting neurons in the hypothalamic preoptic area, GnRH is transported by blood through the hypothalamic-pituitary-gonadial axis to the pituitary gland, which contains gonadotropic cells, with receptive GnRH receptors a seven receptor G-protein coupled transmembrane called gonadotropin-releasing hormone receptor .

The binding of GnRH to GnRHR subsequently activates the synthesis and secretion of gonadotropins luteinizing hormone and follicle-stimulating hormone in the pituitary gland. These processes are controlled by the size and frequency of the GnRH pulses, as well as by androgen and estrogen feedback. Low frequency GnRH pulses lead to FSH secretion, while high GnRH pulses stimulate LH release. GnRH is degraded by proteolysis in a few minutes.

GNRH ACTION RESULTSThere are differences in GnRH secretion between females and males. In males, GnRH is secreted in pulses at a constant frequency however, in women, the pulse rate varies during the menstrual cycle, and there is a large surge of GnRH just before ovulation. GnRH secretion is pulsatile in all vertebrates, and is necessary for proper reproductive function. Therefore, a single hormone, GnRH1, controls a complex process of follicular growth, ovulation, and maintenance of the corpus luteum in the female, and spermatogenesis in the male.

Natruiretic Peptides Nitric Oxide And Guanylyl Cyclases

The natriuretic peptides atrial-, B-type, and C-type natriuretic peptides act via cell surface guanylyl cyclase containing receptors to stimulate cGMP accumulation, which causes activation of protein kinase G and cyclic nucleotide gated ion channels . These are single trans-membrane enzymes which are thought to act as homodimers . There are three subtypes of receptor, NPRA which has high affinity for ANP and BNP, NPRB , which is selective for CNP, and the NPRC receptor which binds all three peptides and acts predominantly as a clearance receptor . The effects of ANP and BNP on hemodynamic and cardiovascular regulation are well documented . The physiological roles of CNP are less clear, although a critical role in endochondral ossification is evident . CNP is expressed in LH positive cells of the anterior pituitary , and female mice with either the CNP or GC-B genes deleted are infertile .

Migration Of Gnrh Neurons During Development

While the majority of neural cells arise from neurons within the developing nervous system itself, GnRH neurons are unusual in that they are derived from progenitor cells in the epithelium of the olfactory placode. These nascent GnRH neurons migrate along the vomeronasal axons, across the cribiform plate and into the mediobasal hypothalamus where migration ceases and the neurons detach from their axonal guides.3 Patients with delayed puberty due to abnormal GnRH neuronal migration frequently have associated anosmia, or the inability to smell, which reflects the fact that the GnRH neurons share common embryonic origins and migratory pathways with olfactory neurons. The identification of genes which direct normal GnRH migration and function is an active area of research. A long list of soluble factors have been identified which appear to be critical for the ultimate development of a network which contains the appropriate number and location of GnRH neurons. These include pathway markers , cell cycle arrest , signaling molecules , growth factors , and adhesion molecules .4 Mutations in these genes result in clinical phenotypes including pubertal delay and infertility, and are discussed in the Section The GnRH receptor below.

What Is The Normal Lh Level In Males

Even though there are different ranges of luteinizing hormone that is considered normal for women, there is only a single range that is used as a reference when testing levels of this particular hormone in a male patients body.

In both women and men, the LH blood test is often used in conjunction with other tests to help diagnose:

  • The cause of infertility
  • Pituitary disorders that can affect LH production
  • Conditions that affect the function of the ovaries or testicles

In Men, high LH levels may indicate a condition affecting the testicles themselves. Some examples include:

  • Viral infection
  • Failure to develop normal gonads
  • Chromosome disorder, such as Klinefelter syndrome

The minimum level should generally be 1.24 IU/L, with a maximum recommended level of 7.8 IU/L.

When levels of LH in the male body declines too much, it can lead to problems, such as hypogonadotropic hypogonadism.

In such cases, testosterone levels may become low and affect fertility and sex drive. When appropriate treatment is initiated in this situation, it may not only improve fertility but also boost sex drive in the patient.

Which Hormones Affect Fertility In Men

Many hormones can have an adverse impact on fertility among male patients. Therefore, men must understand how each of these hormones can affect their fertility. These hormones should also be included in a test when infertility is an issue.

The primary hormones that need to be addressed when looking at how the endocrine system can play a role in fertility among men include:

  • Follicle-stimulating hormone
  • Gonadotropin-releasing hormone
  • Testosterone

Each of these hormones has their own roles to play in fertility with testosterone being at the end, the hormone that ultimately affects that production of sperm.

Negative Feedback Control Of Gnrh

Just about every system and hormone in the body has a kind of off switch: a signal that tells it to slow down or ease up. Often, when a particular hormone increases, its own end result is the off switch. This is called a negative feedback loop, and it keeps the levels of hormones and neurotransmitters within a certain healthy range .

GnRH has multiple off switches. The first is GnRH itself: when neurons release this hormone, they also become less sensitive to stimulation. This may be why GnRH is released in short pulses rather than at a steady rate .

Sex Steroids

The final downstream products of GnRH signaling are the sex steroids, testosterone and estrogen. These, in turn, put the brakes on GnRH production from the hypothalamus .

Progesterone is yet another GnRH off switch. In women, GnRH pulses become very quick right before an egg is released. These fast pulses trigger a huge spike in luteinizing hormone , which in turn causes the ovary to release the egg. Once this happens, the ovary produces progesterone, which then blocks GnRH release .

Reduced Kisspeptin

Stress, calorie restriction, and highprolactin can all decrease GnRH release as well. Kisspeptin, the compound that would normally signal the hypothalamus to make GnRH, decreases in response to these stimuli .

What Is The Hpg Axis

HPG axis The hypothalamic-pituitary-gonadal axis refers to the hypothalamus, pituitary gland, and gonadal glands as if these individual endocrine glands were a single entity. The HPG axis plays a critical role in the development and regulation of several of the bodys systems, such as the reproductive and immune systems. Fluctuations in this axis cause changes in the hormones produced by each gland and have various local and systemic effects in the body. The axis controls development, reproduction, and aging in animals. Gonadotropin-releasing hormone is secreted from the hypothalamus by GnRH-expressing neurons. The anterior portion of the pituitary gland produces luteinizing hormone and follicle-stimulating hormone , and the gonads produce estrogen and testosterone.

How do GnRH analogs work?GnRH analogs are GnRH receptor agonists and work by increasing or decreasing the release of gonadotropins by the pituitary gland and the production of sex hormones by the gonads.

What gland does Gonadorelin and GnRH affect?Gonadotropin-releasing hormone is secreted from the hypothalamus and stimulates gonadotropic cells in the anterior pituitary gland to release luteinizing hormone and follicle-stimulating hormone, which in turn regulate the gametogenic and sterogenic functions of the gonads in men and women.

Does GnRH cause weight gain?Suppressing sex hormone levels with gonadotropin-releasing hormone agonist therapy can cause fat gain.

Interactions Between Kisspeptin Neurokinin B And Dynorphin

Gonadotropin Releasing Hormone Analogues Gnrhas ...

KDNy neurons act synergistically to induce coordinated and pulsatile GnRH secretion by regulating the neuroactivity of other KDNy cells. This is supported by the existence of neurokinin B and kappa opioid peptide receptors within the KNDy cells, but not kisspeptin receptors, which are predominantly expressed on GnRH neurons . Neuron-neuron and neuron-glia communications via gap junctions contribute for the synchronised activities among KNDy neurons .

An increase in the expression of Kiss1 in hypothalamic neurons was observed following Senktide administration , and its stimulatory effects were abolished in Gpr54 knock-out males . In ovariectomised goats, neurokinin B stimulated LH secretion through electrical multi-unit activity corresponding to LH secretion, suggesting a hypothalamic site for this GnRH pulse generation . GnRH antagonists abolished the stimulatory effect of neurokinin B, demonstrating its site of action to be functionally higher than the GnRH receptor .

Studies involving the administration of opioid antagonists to humans have shown stimulatory effects on LH secretion in late follicular and mid-luteal phases , and together with other studies highlight the inhibitory input by dynorphins on kisspeptin signalling, and consequently on GnRH/gonadotropin secretion. Through the stimulatory effects of neurokinin B and kisspeptin, and the inhibitory action of dynorphin, these neuropeptides coordinate pulsatile GnRH and LH secretion .

How To Use Gonadotropin

GnRH agonists

Leuprolide acetate is injected under the skin, while depo leuprolide acetate is injected into a muscle. Leuprolide injections should be given for 10 days to stop ovulation, after which, your fertility specialist will give other medicines that will stimulate ovulation in preparation for assisted reproductive technologies

GnRH antagonists

Cetrorelix is injected subcutaneously . It is available in a 3mg single-dose regimen and a 0.25mg daily dose to be given on stimulation day five or seven. Your doctor will determine which treatment regimen would benefit you best.

Ganirelix Acetate Injection 250 µg is injected subcutaneously once daily on specific days of the menstrual cycle between the use of other infertility medications such as FSH and HCG.

Why Is Gnrh Important

Any failure, whether total or partial, in the production of GnRH can cause the failure in the production of adequate levels of FSH and LH, which in turn can lead to a total or partial failure of pubertal development. This in turn will lead to a lack of secondary sexual development and in most cases, unless treated, infertility.

In men, the lack of gonadotropins will mean the testes will not grow at puberty and they will not produce their own testosterone and sperm.

In women, the lack of gonadotropins will mean the ovaries will not release mature eggs and they will not produce oestrogen or progesterone and menstruation will not occur.

How Is The Test Administered

To perform an LH response to GnRH test, your doctor needs to take a sample of your blood. Then theyll give you a shot of GnRH. Over a period of time, typically 20 minutes and 60 minutes after the injection, additional blood samples will be drawn so the luteinizing hormone can be measured.

Youll do the test in a lab in your doctors building or right in the office. A nurse or medical assistant will take your blood by inserting a needle into a vein in the inside of your arm. A tube connected to that needle will collect a small amount of blood.

    The control of the Hypothalamus pituitary gonadal axis in men is more complex than simple diagrams make it out to be. They often miss out our T à E2 conversion steps, inhibin production and so on. As a recap at the most basic level, the following happens :

  • The hypothalamus, stimulated by the medial preoptic nucleus, secretes Gonadotropin-releasing hormone into the hypophysial portal bloodstream at the medial eminence, which is carried to the pituitary gland. This is pulsatile in nature, ie. it happens at regular intervals.
  • GnRH stimulates the synthesis and secretion of follicle-stimulating hormone and luteinising hormone in the pituitary gland. A low frequency GnRH pulse ensures FSH release, whereas a high-frequency pulse stimulates LH pulses 1.
  • LH stimulates the Leydig cells of the testes to produce Testosterone. FSH acts synergistically here. Intratesticular oestradiol is also produced, as is inhibin, which negatively feeds back to inhibit to production of FSH in the pituitary gland.
  • Both the liver and adipose tissue act as major sites for the conversion of testosterone to oestradiol via the aromatase enzyme.
  • Oestradiol and Testosterone both feedback negatively to inhibit the production of GnRH at the Hypothalamus, as well as LH and FSH production and secretion at the pituitary gland.
