Thyroid nodules in this category are considered to be suspicious for malignancy, and all these nodules should be further investigated with FNAC . The first distinctive feature of these suspicious nodules is their hypo-echogenicity . The echo signals of the nodule or part of the nodule are less than the surrounding normal thyroid tissue and sometimes lower than the nearby muscle . It is important to note that these nodules are hypo-echogenic, but they are also predominantly solid in consistency. This property makes their echo signals higher than those of a cystic nodule, which is dark and echo-free. On a spectrum from highest to lowest likelihood of malignancy, predominantly solid nodules have the highest risk, while mixed solid/cystic sit in the middle, and cystic or spongiform have the lowest risk . Furthermore, the suspicious nodule may have disrupted eggshell calcification around the peripheries or lost its smooth round contour, and adopted a lobulated margin . A U4 thyroid nodule is hypo-echogenic, with an irregular outline and possible disrupted calcification at the edges.

As a point of reference, the information provided is based on my clinical experience treating patients with hypothyroid and subclinical hypothyroidism and some cases of hyperthyroid. It is also based on my medical school training and some additional cursory research to see if there are any research papers documenting how TSH is affected by stress. For the most part, this information is well-established and there is little debate about how stress affects TSH.
TSH also known as thyroid stimulating hormone is affected by stress through the role of cortisol. Anyone with a low thyroid function knows that losing weight feels nearly impossible. Some of us try starving, some of us are exercising excessively despite a distressing lack of progress, and some of us have simply given up. Before I was diagnosed as Hypothyroid, I was doing all of the above.
The thyroid gland produces hormones that ultimately control a persons metabolism, which is the way in which the body burns calories to provide cellular energy. When our thyroid hormone levels are low, calories are not burned effectively, resulting in weight gain even when very few calories are consumed. You might feel as though everyone around you eats normally, without consequence, whereas you eat very little and yet continue to gain weight. Water retention also tends to be high.
Ideally, treatment with thyroid hormone increases an individuals metabolic rate, thereby increasing the amount of calories that are burned which results in weight loss. - No treatment/watchful waiting. If the nodules are not cancerous, you and your doctor may decide that you dont need to be treated at this time. You will see your doctor on a regular basis so he or she can watch for any changes in the nodules.
- Radioactive iodine. Your doctor may use radioactive iodine to treat hyperfunctioning thyroid nodules and goiters with several nodules. In order for thyroid hormone circulating in blood to have a physiological effect, it must first activate receptors on cells. Inflammatory cytokines have been shown to suppress thyroid receptor site sensitivity.
If youre familiar with insulin resistance, where the cells gradually lose their sensitivity to insulin, this is a similar pattern. Its as if the thyroid hormone is knocking on the cells door, but the cells dont answer.
While theres no practical way to measure receptor site sensitivity in a clinical setting, the research above suggests it is decreased in autoimmune and other inflammatory conditions. A perfect example of this in practice is the Hashimotos patient who is taking replacement hormones but still suffers from hypothyroid symptoms often in spite of repeated changes in the dose and type of medication. In these patients, inflammation is depressing thyroid receptor site sensitivity and producing hypothyroid symptoms, even though lab markers like TSH, T4 and T3 may be normal.
